Dear KH,
Yeast and/or parasites do not always cause continuous diarrhea and cramping.
Bacterial overgrowth in the gut (bad bacteria) occurs when the gut environment is out of balance, which can actually result from antibiotics. Too few probiotics (good bacteria), yeast/fungal overgrowth, parasitic toxins, poor diet – there are many avenues.
Just curious, is your Gastroenterologist an Integrative Practitioner? If not, perhaps you can find one nearby at http://www.acam.org or http://www.functionalmedicine.org.
If it were me, I'd not only confirm the SIBO through the breath test, but I'd also get a CSA (see Testing toward top of this forum – you can order it yourself without a doctor) before I considered antibiotics. That's just my opinion. You have to make your decision. To me, more information about what's really going on in the gut is much better than guessing.

I have asked you a couple of questions over the past couple of months. I had been vacationing in South Carolina in June when I developed diarrhea and cramping. I have a history of irritable bowel and multiple food intolerances and am very careful with my diet; however, we were eating out at night so perhaps I got something not good for me. It was also over 100 degrees during the day and I felt like I had heat stroke. My bowels had been in maintenance, regular, and no real complaints prior to the trip, but since that time, they are clearly not normal. When I took your 50 billion probiotic product, it seemed to help. I also cut out eggs, gluten free oatmeal, and gluten free fig bars. Eventually I began to get a bit constipated and you recommended the 30 billion probiotic or Senior Formula. The constipation left, but gradually loose stool and an increase in gassiness have returned. The past few days I have little appetite most of the time. I am trying hard to avoid or limit Fodmaps….challenging since some of them are foods that I actually tolerated well prior to my trip. I decided maybe I had a parasite or a bacteria and went to my doctor to get labwork done. She does not think I have bacteria or parasites because she said I should be continually having diarrhea and cramping. She thinks, though, it is small intestine bacterial overgrowth. I agree this sounds very likely and am surprised that the office now tests for this as I think this is a recent change. My understanding is that most doctor's offices treat SIBO with antibiotics and I wonder your thoughts regarding this. Besides irritable bowel, I have an irritable bladder and sometimes herbal remedies cause discomfort. Your books likely outline how you would treat this, but do you feel I should be wary of taking antibiotics or do you feel if taken concurrently with a good probiotic, it's a good idea? Also, do I likely need another strain of probiotic bacteria I am not getting? Tonight I've taken a 50 billion. I will see the gastroenterologist in a bit over a week and my doctor thinks she'll order the SIBO breath test. Thanks so much for reading this and for your time.
